Community Health Report

The Tri-County Community Health Improvement Coalition has released its first shared Community Health Report, highlighting how wealth, housing, belonging, and access to basic needs shape health across our region. The coalition is comprised of four area health systems, five Public Health agencies, and more than a dozen nonprofit community organizations in the Tri-County area (Calumet, Outagamie, and Winnebago counties). United Way Fox Cities has a deep interest in supporting efforts to create thriving communities and is proud to house the coalition and serve as the backbone entity for this initiative.

Community Health Improvement Plan

A Community Health Improvement Plan (CHIP) is a long-term, systematic plan to address community health issues. The plan is based on the results of assessment activities and the community health improvement planning process. The plan is used to set priorities and coordinate and target resources. A CHIP is critical for developing policies and defining actions to promote community health. It defines the vision for the health of the community.
