    Please fill out the fields below with the necessary information and required attachments. After you have filled out the form, please click the submit button, and it will redirect you to our online payment processor, Municipal Services Bureau (MSB).

    Please select the permit/application types you are submitting. You may select multiple application types at once. This section has no impact on fees that are submitted and is for reference only.

  6. Please include the name of the primary contact person for the permit/application.

  7. Please include an email address for the primary point of contact for the submission. This email will also be used for any additional questions related to the submission and for sending out approval documents. 

  8. Please include either the address of the subject property or the Parcel ID.

  9. Please submit a copy of your signed and completed permit application or soil test for review. Please make sure to include any additional supporting materials. **Multiple documents may be added at once by selecting multiple documents at a time in the document selection popup window.**
